I noticed in the demo that there's only a spot for clients to sign. Is it possible to do the following?

1. Have a client enter their info (name, address, e-mail, phone number)?
2. Have a section for photographer to initial payment schedule when deposit and final payment is received? I like everything on one document.

DETAILS OF PROVIDED SERVICES AND REQUIREMENTS


The Photographer will photograph the event in a candid manner in a format and setting the Photographer deems will make the best photographic shot without considerations from attending guests or family members. Suggestions by the Client(s) will be taken into consideration, but final photographic shots will be at the discretion of the Photographer.
Photographer will take formal portraits as needed and other requested photos as outlined in the "Special Photo Requests" section of this contract.
It is suggested that the Client provide a full list of formal photographs desired; including the names and relationship of individuals the Client wishes to have specifically photographed in their Formals photo session.
Prior to or during the wedding event, the Client may inform the Photographer of subjects which they would like photographed, but the final content of the images will be at the discretion of the Photographer.
It is the Client's responsibility to ensure that the guests and wedding attendees do not interfere with the Photographer's work during the course of the contracted event. All issues arising from interference by others to fulfill the Photographer's contractual obligations will be brought to the Client for resolution.
During the course of the wedding event, the Photographer maintains priority status in all aspects of photographic requests and shooting of photographs, and the Client hereby understands that as a paid service provider, the Photographer is under a time constraint to complete the contracted work and requires time to complete his work in a professional and timely manner without interruption.
While using digitally based photographic processes, the Photographer is under no obligation to display these images during the course of events or after the event until at such time all monies owed for services have been paid in full.
The Client hereby acknowledges that the Photographer will not accept, nor act upon any requests, directions, or instructions for services from others not listed on this contract during the event, without the Client:



First having knowledge of and providing verbal instruction and consent to the Photographer to provide such service.
All requests that fall outside the terms listed in this contract will not be performed by the Photographer unless compensation has first been agreed upon by all involved parties to this contract to perform said services.

TERMS AND CONDITIONS


It is understood this Photographer is the exclusive official Photographer retained to perform photographic services requested on this contract.
Upon payment of deposit and signature of this contract, the Photographer shall reserve the date and time as stated in this contract, and will not make other reservations for this time and date.
The Client hereby understands that all deposits paid are non-refundable. In the event of a change in date, the Photographer reserves the right to consider the contract a cancellation unless the Photographer's schedule permits him to shoot this event on the change in date and time. If such schedule has been reserved for other client events, the Photographer will consider the event as Cancelled by the Client and no monies will be refunded due to the change in contract.
No part of any order, including Proofs, Digital Files, or other media will be delivered until the balance of the contract has been paid in Full.
The Photographer reserves the right to use all digital images from your event  for normal advertising, web and Social media display, publication, or other purposes that apply to the promotion of Photographer's services. The Photographer is exclusive owner and copyright holder of all images retained from this event, and duplication, copying, modification, displaying, and printing of copyrighted materials is prohibited without express written consent from the Photographer.
In the event of the Photographer's illness, emergency, or other issues arising that are beyond the control of the Photographer; the Photographer hereby reserves the right to substitute the event photographer with another competent photographer of equal or greater skill and talent to photograph the event outlined in this contract.
If the Photographer cannot perform this contract, nor supply the Client with another competent photographer due to fire or other casualty, strike, act of God, or other causes beyond the control of the Photographer, the Photographer agrees to refund any deposits made by the Client but shall have no further liability with respect to this contract. This limitation on liability shall also apply in the event that photographic files and materials are damaged in processing, lost through camera or other hardware and media malfunction, lost/damaged in the mail, or otherwise lost or damaged without fault on part of the Photographer.
In the event the Photographer fails to perform for any reason, the Photographer shall not be liable for any damages or monetary amounts in excess of the monies paid to date by the Client.
Client understands and is aware that materials used in the printing process may fade or discolor over time due to inherent qualities of pigment inks and dyes, and Client hereby releases Photographer from any liability and claims whatsoever based upon fading or discoloration of any finished prints the client may purchase from either Photographer or other outside parties.
The charges in this contract are based upon the Photographer's current pricing structure and may change at anytime without notice. The Photographer cannot guarantee prices listed in this contract will remain the same for future bookings and orders. All future orders and pricing shall be charged at the prices in effect at time of purchase.

Litigation & Legal Action:

Any actions made against the Photographer may be taken before a mediator at the election of the Photographer in lieu of court or civil suit. All rulings against Photographer for breach of contract which result in monetary damages awarded shall not exceed the total amount of monies paid to Photographer by Client, or otherwise specified in this contract.

NOTICE OF COPYRIGHT:

All images or work produced for this event remain the sole property and copyright of the Photographer. Client further agrees and acknowledges no claim to photos or other items produced by Photographer. Client further acknowledges and agrees that all photographs and other works produced by Photographer may not be copied, reproduced, modified, or displayed elsewhere without Photographer's written permission and consent. Violators of this Federal Law will be subject to its civil and criminal penalties and will be pursued to the full extent of the law.

PAYMENT TERMS

By signing this agreement, the Client hereby agrees to pay the following amounts:

All payments may be made by credit card, PayPal, cash, or check drawn on a U.S. bank. No payment will be accepted from checks drawn on banks from outside the United States. Payments made by check must process my bank prior to day of event for services to be rendered. No services will be performed in the event of non-payment, no exceptions will be allowed.

This contract incorporates the entire understanding of the parties listed and is governed by the laws in the State of Kentucky. All modifications of this contract must be done in writing and signed by all parties involved.

Signing below you state that you have read and understand this agreement, including the terms and conditions, and agree to be bound by them.
