I get an error message when tryng to process my cvs file with passcodes.
I have uploaded an image of the message and a print screen of my cvs file.

It is probably the formatting or encoding of the CSV file. Try opening that file in a text editor like notepad and resaving it, then upload it and see if it makes a difference.
