Contact Form Error

Clients are getting this message when they fill out my contact form on my site:

Sorry, looks like something isn't right. If you continue to have issues, please email us code: BOTSESS

This is happening because you actually have 2 forms on that page and making it not pass the new bot check. 

You have the one you see and there is another one you tried to add with the page designer but you never selected a form to add. That is why you have this black box on the page.

Edit that page with the page designer, hold your mouse over that section then click delete row and save.
