Qr Gallery Doesn't Even Scan A Screen Shot Of A Qr Code, But It Scans On Phone

Of course if a QR code is blurry or partially covered or bent it won't scan properly.  We do our best but at a school we shot last week I got about 20% fail rate on the uploads. 

After a certain amount of cursing at my photographers I decided to make screen shots of the actual QRs and paste them into the photos so I could keep the filenames and metadata but theoretically guarantee that the QR is perfect and will scan easily.

There are still 10% of the cards that will not scan!! 

Attached is an example of one that did not, the others are similar.  They are 2400x3000px so it's a decent size, I have tried everything I can think of including making it B&W and increasing contrast to make all the edges neat and perfect.  Still, in my school of 900 students I have to go through and manually re-code roughly 90 sessions. 

What makes it worse is that the code scans perfectly on our phones -- you can scan this code to see the photos in the gallery with no issues.  

What is happening?  Is the Sytist QR scan broken?  Anyone else with this issue?

I think it has something to do with the checks in the shirt or the black & white.
