Rogue "payment Form" Showing Up On Every Page

Every time I click on a page, I get a "Payment Form" popup.. Did I get hacked? Or is there some setting that I accidentally changed that created this? It goes away by clicking the "Complete Payment" button even if all the fields are blank or fake info. Please help!

That should not be there, looks like you have been hacked somewhere.
You can email me the following for me to look into it but probably happened from another part of your site.
1) The link / URL to your Sytist admin (where you log into your Sytist admin).
2) The admin username and password (or create me an admin account by clicking Admins in the upper right corner of the admin pages).
3) FTP username and password.
