Those options are not showing on the order at all in the admin view? Where purchases made from galleries or products from different price lists?

Are there any actual photos on the order? Or if a pre-order gallery, any photo products on the order? It checks for additional checkout fields based on photos on the order (or photo products for pre-orders).

I have tested every scenario I can think of and the options always show in my testing. You would need to email me the link to your Sytist admin with the username and password and the order number of an order the additional fields were not added so I can look it over.
