Adding The Find My Photos Form Into A Page

I want to be able to add the find my photos form to a page so clients can be directed to that page to enter their password. Currently the only options are either on the home page or via menu link. I want to be able to do it from a specific but simple page link like www.milnerstudios.com/view

I remember using this embed code last year and it worked perfectly. I just came to use it again, and it doesn't look like the code is still there to copy? :-(
