Passcoding Sub Galleries

Can someone help me with how my CSV needs to be for passwords in subgalleries?

School>bldgA>Kid1 (abc.jpg, abd.jpg, abe.jpg), Kid 2 (abc.jpg, abd.jpg, abe.jpg), kid 3 (abc.jpg, abd.jpg, abe.jpg), etc.
There was a main folder uploaded, which had four folders inside it, and each of those four folders have a folder for each individual containing their images. I am wanting a single password for each individual, OR, one password for each of the four folders.

The images/subfolders are already uploaded.

I went to the FAQs and support pages and found the article on it, but it would not let me download the sample CSV. I am just wanting to see how the rows and columns are to be set up to do this.

You can download that CSV file example here. Attached is a screenshot of the CSV file. Not sure why the other link isn't working.

Unzip it and you will find the example in  there.
