How To Add Input Box In Appointment ?

How can we add an input box or multiple choice check box for a client to answer when they make an appointment? I am talking about a box or input for "type" of session, how many people, comments or special requests, how did you learn about the studio , etc..

I am having the email them after they book online in a separate email to ask all these when they could have given me all that on the appointment page and included in the email we get.
