Add A Form To My Booking To Collect Address Info

Tim I have created a form to collect my client's information for doing real estate shoots. I need to collect the property address information during their booking. If I add the form to the booking page will this flow through in the booking email I receive or will it be a separate email sent by the form routine? If separate, is there a better way to accomplish this so that all the booking info remains together?
I suppose I could add this as an options field for every type of booking I offer but was hoping to use the [FORM] feature in conjunction with the booking app.

A form does not work with the booking. It is completely separate. You can add options to your booking service (the options link when editing the booking service) and add an option to ask for the address.
