Form Email Sender Subject Switched To Give Me No Text Information

HELP!!! I have created two different forms and named them differently to track where my customers are signing up for photos from.
One of the forms was giving me the school name as the sender which is what I want.
The other is giving me the child's name which I do not want.
I tried to make the forms exactly the same and now I messed them up and they are both wrong. LOL
HELP! How do I go back to being able to make the school name text box the sender name in my email. This is happening for gmail and outlook so I know I have a setting wrong somewhere in stytist.

It will send "from name" for the field that is marked as a name field. You should only have 1 name field per form.
