Calendar Not Limiting One Booking Per Time Slot

I've had coaches signing up for time slots for a large soccer league that we will be shooting on April 7 and April 14. Even though I set appointments per time set to 1, I've had several time slots where there are 2 or 3 coaches who have already signed up for same time slot.

I need this fixed ASAP so I don't get too many teams signed up too close together. Why is it not limiting time slots to one booking?

How far apart in time were the slots booked for the multiple bookings?

If you view that on the website where they are booking, does it show the time available that has been booked?

Yes. All the times are all still available, even those that have been booked by multiple teams.

Here is link to the book now page and when calendar opens every time slot on April 7th is open even though 40plus teams have booked a time.

It looks like it resolved itself when I added option of making coaches give me the team name yesterday. See the list of teams signed up attached. There are many that have the same time slot. However, now that team Butterflies 12 signed up for the 9:30 a.m. slot that time is not longer available on the calendar. However, as you can see with other time slots being duplicated, that was not the case earlier.

The ones with the same time slots, did they book like within minutes of each other? Adding an option for the team name wouldn't have any effect on the availability of the time slot.
