Pre-registration Form Options

Is it possible to add a few extra form fields to the gallery pre-registration form?

Hi Tim,
Any chance this could be made an option? I usually send out a google form (to the school, they forward it to parents) prior to releasing galleries to collect email addresses. It includes a model release so I can get parents excited by posting a couple of photos on social media. It would be awesome if I could do this in the sytist preregistration instead!
