You can create forms to add to pages that your visitors can fill out and the results are emailed to you. Most common use of a form is a contact form.
Once you create a form you will need to add it to a page. Create the page you want it to be on in the admin if you haven't already (like a top level page). Add [FORM] into the text editor where you want the form to display. The then click the "more options" link, go down to Page Forms and select the for to show on the page.
*** Starting with Sytist version 2.5, you can also add a form to a page using the advanced page design option. While in the page designer, click add design element and from the categories select the dynamic content/forms option.

Email results to
When the form is submitted, it will email the results to the email address you enter here. So enter in your email address here. If you want the results to be sent to more than one email address, enter in the email addresses separated by a comma. firstname.lastname@example.org,email@example.com
This is the subject of the email when you receive the results. Keep in mind that is you reply to one of these emails, it is still in the subject line.

Form Success Message
When someone successfully fills out the form and sends, this is the text that is shown on the page.
Adding Form Fields
Once you have created or edit a form, you will need to add form fields. These are the things they are going to enter or select. Select from the dropdown menu in the Add new form field section the type of field you want to add.

This is a standard one like area to enter text
When creating a form it is important to add an email address field so you know who to reply to. The field will look just like the text field.
This is the larger area for text / messages.
This is the menu that opens up with multiple options to choose from. When you add a dropdown field you have to enter in the options for them to select in the Options section.
Radio buttons are also used for multiple options but are just shown on the page instead of a dropdown menu that opens up. This is useful for yes/no questions.
A checkbox give a single box to check for a question.
Info / Text
This is used if you want to add just text between form fields. There is no form field option with this one.
