Build A Form From My Pdf Copy

Hello I was wondering if I am able to create this form on the website and have it place the content into an excel file on the back end. Based on the Team name.

You can create forms in the Form section but I don't think you can match that one. And they don't export into a spreadsheet.

Wouldn't this be something that Google forms or Microsoft forms is absolutely ideal for?

You might be then able to embed the form in your site.
