Trying To Add A Store Page And Having Issues

I'm trying to add two options as a store page that looks like:

But I can only come as close as:
https://www.terryyoungphoto.com/image_upgrades/upgrade_webfile_to_high_res/
My form always has an extra "Add to Cart" Button I don't won't as I'd like to use the Qty: Box Add to Cart of the store page layout

It looks like you added a form ( like a contact form) with the page designer to that page. You need to remove that.
And you can add the options to the product using the options link when editing it in the admin.

Yes I added the form to get the extra fields i need filled in. I'll try the options link.
