We want to set up our booking calendar in a way that a client is required to set up an account (or log in to their existing account) in order to book a shoot through our online booking calendar. How / where do I set that up?

There isn't a way currently to force someone to create an account to book a session UNLESS the session requires a deposit. Then they would create an account at checkout.

Is the "Create an Account" form/page automatically created by sytist, or do I need to set that up? That's actually what I am looking for. I'm sorry for the confusion.

They will create an account during the checkout process. The form is automatically there. And you can make sure they do by going to Settings -> Checkout & Payment in your Sytist admin, go down to the "Customer Account Options On Checkout" section and check the "Require create an account" option.
